Minnesota Small Employer Mandated Plan Options
Minnesota's Small Employer Insurance Reform legislation (Section 62L) requires that all health insurance carriers notify small employers (2-50 employees) of the availability of state-mandated health plan options at renewal. Employers are not required to select any of these options; this information is simply provided for your reference. If you would like more information on these plan options, please contact your broker or Medica sales representative.

Medica Insurance Company Options
Note: These plans are only available in the Medica Insurance Company (Medica) MN selling service area. Please contact your broker or Medica representative if you have questions regarding our MN selling service area.
Option 1: Small Employer Copayment Plan MICMan Copay — Network and Non-Network
This plan offers 80 percent coverage, after a copayment has been applied, for most eligible services; and 50 percent coverage for prescription drugs. In-network preventive services are covered at 100%. There is also 50 percent coverage, after a $4,500 deductible has been met, for non-emergency services received from non-network providers.
Option 2: Small Employer Deductible Plan MICMan Ded
This plan offers 80 percent coverage, after a $2,250 deductible has been met, for most eligible services; and 50 percent coverage, after the deductible has been met, for prescription drugs. In-network preventive services are covered at 100%. The lifetime maximum benefit is $1,000,000 per member. There is also 50 percent coverage, after a $4,500 deductible has been met, for non-emergency services received from non-network providers.
