Medica Choice® Passport members have access to the large Medica Choice network when they receive care within our service area ___ Minnesota, North Dakota, South Dakota and Wisconsin. Those located outside our service area have access to a large national network of more than 600,000 providers and 5,000 hospitals. To determine which network applies to a particular zip code, use the Passport Zip Code Look-up Tool.
The national network is made up of two provider networks:
Large groups with Preferred Portfolio Options use the UnitedHealthcare Choice Plus network.
Large groups with 150+ employees with a specified benefit configuration may use the UnitedHealthcare Options PPO network.
Small groups use the UnitedHealthcare Choice Plus Network.
Members can search our list of preferred drugs (formulary) to find out if their prescriptions are included, or to look for generic alternatives to existing prescriptions.
