Network Information
Medica Choice®Passport members have access to the large Medica Choice network when they receive care within our service area — Minnesota, North Dakota, South Dakota and Wisconsin. Those located outside our service area have access to a large national network of more than 600,000 providers and 5,000 hospitals. The national network is made up of two provider networks:
Medica Choice UnitedHealthcare Choice Plus Network
- Large groups with Preferred Portfolio Options use the UnitedHealthcare Choice Plus network.
- Small groups use the UnitedHealthcare Choice Plus Network.

Medica Choice UnitedHealthcare Options PPO Network
- Large groups with 150+ employees with a specified benefit configuration may use the UnitedHealthcare Options PPO network.
